Walk-Ins Welcome
Please bring the following items whenever possible:
A valid government-issued photo ID
Your insurance card (if using insurance)
A payment method for any applicable copay or self-pay charges
A current list of medications, if applicable
Relevant medical records or recent test results, if available
School, sports, employment, or other required paperwork if you're visiting for a physical
Workers' compensation claim information, if your visit is work-related
